Meetings

• Endocrinology virtual clinical champion meeting is Thursday, November 6th from 6pm-8pm. There is no registration for this meeting but a calendar invite has been sent to all endocrinology clinical champions and PO Admin Leads and Primary Contacts.
• Second and final virtual fall regional meeting is taking place on Thursday, November 13th from 6pm-8pm. If you had clinical champions who missed the first meeting, please have them attend this one.
• PO Monthly Calls are taking place on November 10th at 11am and November 12th at 2pm.
• Learning Community Event: Pregnancy and Type 2 Diabetes is taking place on Friday, November 14th at 12pm, and will be led by Dr. Jen Wyckoff.

VBR Reports

• Performance measure reporting will be distributed in November for the PCP uACR measure, the PCP A1C measure, the nephrology uACR measure, and the endocrinology CGM interpretation measure.
• We anticipate being able to update the PCP A1C measure with targets in November, once we receive the updated 90th percentile rates from HEDIS.

Reminders

• Practice census surveys for PCP practices are due December 1st. Please support your practices in completing these.
• Specialists need to log into the patient data dashboards once before the end of the year. POs should have received an email if their specialists still need to be granted access to the dashboard.
• Any practice that completes both the physician level and practice level learning community requirements before 1/1/2026 is eligible for 5 extra credit points on the value based reimbursement scorecard, so please encourage your practices to take advantage of this!
